NREMT Paramedic Exam Prep: a practice course for the National Registry Paramedic cognitive exam — a computer-adaptive test of 110 to 150 questions in 3.5 hours, drawn from the ALS test plan in effect since July 1, 2024, with Clinical Judgment the largest domain (34-38%). Passing is an entry-level competency determination by the adaptive algorithm; the National Registry publishes no percentage score. Since July 1, 2024 there is no separate national psychomotor exam at this level — the cognitive exam is the only national examination component.
